Approche globale Holistic Approach
Why psychopharmacology and psychological support remain unmet needs in everyday dermatology.
Up to one-third of dermatology patients are estimated to have an associated psychiatric comorbidity. A holistic approach — combining dermatologic treatment, targeted psychopharmacology when needed, and psychological support — improves clinical outcomes and perceived quality of life, a principle now widely accepted in the specialized literature.
A 2024 narrative review in CNS Drugs, specifically addressing unmet needs in psychodermatology, notes that this theoretical consensus does not systematically translate into everyday clinical practice — identification and referral of affected patients remaining uneven depending on the care setting and practitioner awareness of this dimension.
A 2025 review on psychopharmacology in dermatology details how antidepressants, anxiolytics, and antipsychotics are used to treat dermatologic conditions with an associated psychiatric component — with demonstrated impact on both skin health and concurrent psychiatric symptoms, but also skin-specific side effects of certain psychotropics that clinicians should be aware of.
Une coordination interdisciplinaire qui reste à construire Interdisciplinary coordination still to be built
Jafferany’s reference guide on major psychocutaneous disorders highlights a difficulty specific to this field: affected patients often lack insight into the psychiatric origin of their symptoms, complicating referral and making them, according to the author, some of the most challenging cases to treat in everyday dermatologic practice.
Ce que cela suppose organisationnellement What this requires organizationally
This coordination remains a real organizational challenge: it requires smooth referral between dermatology, psychiatry, and psychology, still unevenly available depending on the care setting — an issue even more pressing in contexts where mental health care provision is itself limited, as is the case in many regions.
En pratique In practice
An effective holistic approach therefore requires, at minimum, systematic screening for psychological distress associated with chronic dermatoses (using validated tools such as the DLQI), basic knowledge of the indications and interactions of psychopharmacology usable in dermatology, and identification of mental health contacts to whom patients whose care exceeds the scope of the dermatology consultation alone can be referred.
